Trimix (sometimes also noted as Tri-mix) is used to treat erectile dysfunction (ED) in men and is self-administered by injection into the penis. Trimix is considered one of the most effective treatments for ED and is part of a powerful line of intracavernosal anti-erectile dysfunction agents that includes Bimix, Trimix, and Quadmix formulations.

Trimix is a compounded medication consisting of three ingredients in a sterile injection dosage form. Trimix is available in both lyophilized (freeze-dried) and liquid (pre-mixed) forms. Lyophilized Trimix has a longer shelf life and does not require refrigeration until it is diluted prior to administration.

● Papaverine – A potent vasodilator that increases blood flow to the penis.
● Phentolamine – A potent vasodilator that increases blood flow and cardiac output.
● Prostaglandin – An ingredient that increases blood flow and arterial velocity into the penis and increases retention.

Trimix injections are compounded under USP 797 guidelines for sterile compounding. All final products are tested for sterility, endotoxin, potency, and pH prior to being dispensed to the patient.

What is Bimix?

Bimix is a compounded medication similar to Trimix but containing two active ingredients and no prostaglandin. Bimix consists of papaverine and phentolamine in multiple strength options.

Bimix can be a good option for men who have not used intracavernosal injection (ICI) therapy before, who have mild to moderate ED but are not responsive to oral therapy, or who experience side effects from prostaglandin.

What is Quadmix?

Quadmix is a compounded medication containing the same ingredients as Trimix with atropine as the fourth ingredient. Atropine influences smooth muscle relaxation and reportedly increases the effects of the other ingredients. Quadmix is available in multiple strengths and combinations. Quadmix is usually prescribed in long-term ICI users who no longer respond to Trimix.

Trimix is a powerful anti-erectile dysfunction treatment that should be used carefully according to doctor and pharmacy instructions. Reasons Not to Take Trimix

Trimix or ICI therapy is not recommended in the following cases. If any of these conditions apply, you may be referred to a urology specialist for further evaluation and to identify the best options for you.

● Hypersensitivity or allergy to any ingredient
● Conditions predisposing you to priapism
    ○ Sickle cell anemia
    ○ Multiple myeloma or leukemia
    ○ Anatomical deformation of the penis or penile implants
● Direction by your physician that sexual activity is inadvisable or contraindicated
● Trimix is not recommended in patients who respond well to other treatments including oral therapy. Long-term use can increase tolerance and scar tissue.

Warnings: Tell your doctor if you have a condition or are taking a medicine that interferes with blood clotting.

How to Treat Priapism

Trimix and ICP patients should have reversal options on hand for priapism, a persistent and painful erection lasting more than 4 hours. A self-administered rescue kit containing a low dose phenylephrine HCL injection can be used to reverse priapism. If self-administered treatments do not work, and the erection has been present for 4 hours and/or is extremely painful, then immediately visit the Emergency Room for treatment.
